Dreamheart (and the basic "physics" of how the plane of dreams works) is from the Manual of the Planes, a D&D sourcebook. So's Ana as the only known permanent resident of her dreamscape. Can't take credit.
Theodore was human. Still is in a lot of ways, but... a lot's happened to him and there are some subjects on which he doesn't *think* like a human anymore. Space, Time and Thought being the primary ones. He does still have a body though; he usually doesn't dream anymore while he sleeps, and when he does he enters others' dreamscapes rather than one of his own.
